Social Worker

To effectively continue post-discharge patient care, social workers evaluate the continuing needs of each patient and their social surroundings. This is done through extensive patient and family interviews, interdisciplinary communication, and chart screening. Social workers team with other clinical departments to create the most optimal plans of care, and they provide additional resources and services to patients who have little or no support system.  

Job Responsibilities

  • Interviews patients and their families to create goals and understand support systems
  • Corresponds with other departments for post-discharge planning 
  • Conducts psychosocial evaluations and comprises corresponding reports 
  • Aids in patient plan of care 
  • Maintains patient social work paperwork and documentation 
  • Arranges at-home care and maintains relationships with patients and their support system

Qualifications and Skills 

  • High school diploma, GED, or equivalent experience 
  • Bachelor’s degree in Social Work or related field 
  • Master’s degree in Social Work 
  • 2+ years of social work experience   
  • Active state licensure for social work 

Social workers are required to possess strong communication skills to handle traumatic patient situations and provide crisis intervention to specific cases. They must also maintain extreme professionalism with each case while communicating with patients and families.
