Social Worker

From discharge planning to post-discharge counseling, social workers are heavily integrated in health systems. Collaborating with care teams, social workers help patients and families solve problems and access resources. Social workers identify useful community tools, such as financial assistance programs, and educate clients on how to use and locate them. In addition, social workers may aid patients by arranging transportation, scheduling treatments, and coordinating medical equipment access at home.

Job Responsibilities                                                                                                        

  • Help patients and their families access community resources
  • Identify and understand case needs
  • Interview patients and families to understand needs, dynamics, and current support systems
  • Create case plans on an individual basis
  • Document patient sessions
  • Assist with discharge planning
  • Arrange patient transportation

Qualifications and Skills

  • High school diploma, GED, or equivalent experience
  • Bachelor’s degree in social work (BSW) or related field required
  • Master’s degree in social work (MSW) preferred
  • 2+ years of clinical counseling experience
  • Proficiency with word processing software
