Complex Case Management Nurses facilitate care for patients with complex issues, such as mental health or substance abuse. A Complex Case Management Nurse must gauge the physical, social, and environmental needs in these patient cases. Quality of patient care is evaluated in order to form and implement plans to correct or maintain risk management. Complex Case Management Nurses also serve as advocates for patients and families to ensure that communication amongst providers is clear.

Job Responsibilities

  • Create care plans that include treatment objectives, goals, and actions to meet patient needs
  • Conduct remote clinical assessments and use clinical interviewing to address patient wellness
  • Intervene in communication situations between patients and other care providers
  • Evaluate patient progress towards treatment goals and reassess for necessary changes
  • Research alternative treatment options and match patients with appropriate providers
  • Understand patient support and determine caregiver/community resources


Qualifications and Skills

  • Active RN license
  • Clinical nursing care experience
  • BSN or MSN preferred
  • Case Management Certification (CCM) preferred
  • Electronic Medical Record (EMR) experience preferred

To be successful in this role, a Complex Case Management Nurse must be knowledgeable of chronic disease management, patient and family care concepts, and home model of care. Similarly, a Complex Case Management Nurse must be able to work flexible hours, prioritize tasks, and demonstrate exceptional patient service skills.
